SHERMAN OAKS, CA — November 1, 2026 Tomas Venancio Medel, 60, lost his life in a multi-vehicle collision early Monday morning on the San Diego (405) Freeway in Sherman Oaks, according to the California Highway Patrol (CHP).

Authorities reported that the crash occurred at approximately 1:45 a.m. on the rain-soaked freeway. Medel was reportedly ejected from a vehicle that overturned during the collision. He was pronounced dead at the scene.

Two additional people sustained injuries and were transported to a nearby hospital for treatment. The circumstances surrounding the collision remain under investigation.

Incident Location

CRASH INVESTIGATION NEEDED

The California Highway Patrol is examining the circumstances that led to the fatal multi-vehicle collision, including the events surrounding the vehicle rollover and the conditions on the freeway at the time.

Investigators are expected to review available evidence, vehicle positions, roadway conditions and witness accounts to establish how the collision occurred and whether any contributing factors played a role. The official findings will help clarify the sequence of events leading to Medel’s death.

Candice Bond, Managing Partner at Bond Legal and national legal analyst for Accident Injury News, notes that thorough investigations are particularly important in serious and fatal roadway collisions.

According to Bond, in cases such as this, investigators may need to examine physical evidence, interview witnesses and review available surveillance footage to develop a clearer understanding of the incident. Vehicle damage, roadway markings and other available records may also help establish how the collision unfolded.

She emphasizes that preserving relevant evidence and carefully examining the available information are important steps in determining the circumstances surrounding a fatal accident.

WRONGFUL DEATH CLAIM

The death of Tomas Venancio Medel may give rise to a wrongful death claim, depending on the findings of the investigation and the applicable circumstances.

Stefano Formica, Of Counsel at Bond Legal and national legal analyst for Accident Injury News, explains that wrongful death cases can involve damages associated with the loss of companionship, emotional suffering and other losses experienced by surviving family members.

In cases such as this, Formica notes that the official investigation can provide important information about the circumstances of the collision and help establish the facts relevant to any potential wrongful death proceedings.

The CHP investigation remains ongoing, and further details may become available as authorities continue reviewing the crash.
