Michael Faulkner Dies in Fatal Head-On Crash in O

WARREN TOWNSHIP, OH — October 10, 2026 Michael Faulkner, 62, of Rayland, died Saturday morning after his pickup truck was struck head-on by another vehicle on Ohio State Route 150 in Warren Township, according to the Ohio State Highway Patrol (OSHP).

Troopers reported that the collision occurred at approximately 10:53 a.m. and involved a 2011 Nissan Titan traveling eastbound and a 1982 GMC C/K pickup traveling westbound.

According to preliminary information, the Nissan crossed the center line and entered the opposing lane, colliding with the GMC. Following the impact, the Nissan left the roadway on the left side and entered a ditch.

Faulkner, who was driving the GMC pickup, was pronounced dead at the scene. The Nissan’s driver, a 52-year-old man, was transported to WVU Medicine Wheeling Hospital, where he later died.

HEAD-ON CRASH INVESTIGATION

Head-on collisions can cause catastrophic injuries because the impact involves vehicles traveling toward one another. Investigators will need to establish why the Nissan entered the opposing lane and determine whether driver behavior, roadway conditions, visibility or other factors contributed to the collision.

Candice Bond, an experienced auto accident attorney and managing partner at Bond Legal, has highlighted the importance of examining road conditions, visibility and lighting when evaluating serious collisions. In cases such as this, a thorough investigation may also involve reviewing vehicle damage, physical evidence, witness accounts and any available photographs or video footage.

Accident reconstruction specialists may help investigators determine the vehicles’ movements before impact and establish how the collision occurred. A comprehensive review can help clarify the sequence of events and identify any contributing factors.

Although the Ohio State Highway Patrol is conducting the official investigation, an independent legal review may also help establish whether negligence played a role. Such a review can include examining relevant records and preserving evidence that may be important in any subsequent civil proceedings. The cause of this crash and any potential liability have not been established in the information currently available.

WRONGFUL DEATH CLAIM

Following a fatal collision, eligible surviving family members may have grounds to pursue a wrongful death claim if evidence establishes that another party’s negligence caused the death. The availability of a claim and the damages that may be recovered depend on the circumstances and applicable Ohio law.

Stefano Formica, Of Counsel at Bond Legal, has discussed the significant emotional and practical difficulties families can face after losing a loved one in an accident. In cases such as this, families may need to address funeral expenses, financial losses and the long-term effects of their loss while the investigation continues.

A wrongful death attorney can review the available evidence, explain the family’s legal options and communicate with insurers when appropriate. Depending on the facts and applicable law, potential damages may include eligible financial losses and compensation for certain losses suffered by surviving family members.

Any civil claim would require an assessment of the evidence, the parties’ respective responsibilities and the legal requirements for recovery. No finding of negligence or civil liability has been reported in connection with this collision.
