Head-on accident in Chili NY kills Hashini Herath, MCSO reports

CHILI, NY, (April 20, 2026) – A 29-year-old Rochester woman was killed Monday evening in a head-on collision on Scottsville Road in the Town of Chili, according to the Monroe County Sheriff’s Office.

Authorities said a truck traveling south between Milewood Road and Morgan Road attempted to pass another southbound vehicle and entered the northbound lane at approximately 6:45 p.m. The truck then collided head-on with a northbound vehicle driven by Hashini Herath.

Herath was pronounced dead at the scene. A passenger in her vehicle suffered non-life-threatening injuries and was taken to a hospital before being released. The truck driver and a passenger in that vehicle sustained minor injuries.

Officials said the truck driver was issued multiple traffic citations. Investigators noted there was no indication of impairment at the time of the crash.

A fundraiser has been established to help return Herath’s remains to Sri Lanka for burial.

The Monroe County Sheriff’s Office continues to investigate the incident.


CRASH INVESTIGATION

Passing-related crashes remain a significant cause of roadway fatalities, particularly on undivided highways. According to the National Highway Traffic Safety Administration, improper passing contributed to more than 4,000 deadly crashes in the U.S. from 2017 to 2021.

These types of incidents frequently involve head-on collisions and often occur when a driver incorrectly estimates the speed or distance of approaching traffic while attempting to pass another vehicle. The Federal Highway Administration adds that rural two-lane roads are especially vulnerable due to limited sight distance and high-speed traffic.

“Road conditions, visibility, and lighting are all possible contributing factors that must be carefully reviewed,” said Candice Bond, a Managing Partner with Bond Legal.
