MONTEREY TOWNSHIP, Ohio — Authorities are investigating a fatal accident that happened May 5 at the intersection of U.S. Highway 224 and Ohio 66.
The Ohio State Highway Patrol responded to the scene near Ottoville just before 11 a.m.
A 2007 Ford E-150 Van traveling southbound failed to stop at a posted stop sign and struck an eastbound 2021 Honda CR-V. The impact sent the Honda off the right side of the roadway, where it hit a stop sign and overturned. The Ford went off the left side of the roadway.
Dorothy Smith, 72, suffered fatal injuries. Emergency responders pronounced her dead at the scene and transported her to the Lucas County Coroner’s Office. A 72-year-old male passenger in the Honda sustained life-threatening injuries and was taken to St. Rita’s Medical Center in Lima.
Officials reported that all occupants were wearing seat belts. Three children in the Ford, ages 4, 4, and 3 months, were uninjured but transported for precautionary evaluation.
Authorities continue to investigate the circumstances surrounding the accident.
