LEBANON, OH, (April 21, 2026) – A multi-vehicle crash early Tuesday morning in Lebanon left a Middletown man and two others seriously injured, according to the Ohio State Highway Patrol (OSHP).
Authorities said the collision occurred shortly before 4 a.m. near Arrow Springs Boulevard along Ohio 48. A southbound Chevrolet Suburban reportedly veered out of its lane, crossed the median, and struck a northbound Chevrolet Traverse. The Suburban then hit a 2012 Honda Civic before overturning and coming to rest across the roadway.
Dale Peck, 54, sustained serious injuries in the crash and was transported to TriHealth Bethesda North Hospital for treatment. Another motorist in the Traverse was also seriously injured and taken to Atrium Medical Center.
Officials stated that the driver of the Suburban fled the scene on foot following the crash. Law enforcement later located the individual at a medical facility, and he was subsequently airlifted to a hospital for evaluation of minor injuries.
The Ohio State Highway Patrol is continuing its investigation. No charges have been filed at this time.
